A Name Change is the legal act by an adult or minor, of adopting a name different from their name at birth, marriage or adoption. The first step in obtaining a court order to change a name is to file a “Petition for a Change of Name” in the Superior Court. Then, a document called an “Order to Show Cause for Change of Name” is published in a newspaper of general circulation once a week for 4 weeks in a row. A court hearing is then held and if the judge approves your request for a change of name, the judge will sign the “Decree Changing Name”. You can use a certified copy of this decree to change all your legal documents, including your birth certificate, social security card, and other government-issued identification like your passport or driver’s license.
